Title: Improving Health Outcomes in Native American Communities

Summary: This presentation will focus on the prevalence of chronic disease in Native American communities and the importance of managing these conditions to improve health outcomes. We will explore the role of member coaching and health promotion in managing chronic diseases such as diabetes, cardiovascular disease, and chronic kidney disease. Renalogic nurses serve as member coaches, providing guidance and support to help members better manage their conditions. By improving the health of our community members, we can also lower healthcare expenses, making this a win-win strategy for both members and the health plan. We will discuss the challenges and opportunities of implementing such a program in Native American communities and share insights and lessons from our experience.
